Como Park Animal Hospital and Laser Surgery Center is pleased to provide you with a variety of forms that can simplify the process of checking in a new pet, admitting a pet for a surgical procedure, or admitting a pet for boarding.

If you wish to obtain copies of your pet's medical records, please call our office at 651-487-3255, and we would be happy to assist you. Please note, there is a medical records release form that must be signed by the legal owner of the patient in order for records to be released.

Authorization for Anesthesia /Treatment Form	Fill this form out if you will be admitting your pet for a medical or surgical procedure that will require anesthesia (examples: Dental cleaning, Neuter, Ovariohysterectomy--aka: Spay, Tumor removal).
Boarding Admission Form	Fill this form out if you will be admitting your pet for boarding at our clinic.
New Client / Patient Form	Each new patient to Como Park Animal Hospital will need this form for their records. Please fill out one for each new pet and bring it with you to your first appointment.
